A. Affidavit, Special Power of Attorney (SPA), Deeds & other documents requiring acknowledgment

Requirements:

Personal appearance of principal/s, affiant/s or signatories;
Original and photocopy of valid passport;
Payment of the application fee of €22.50.

 

B. Authentication

Requirements:

Presentation of documents with the original stamp of the Procura, Prefettura or the Camera di Commercio in places under the jurisdiction of the Philippine Consulate General;
Documents may be filed by an agent, or sent through a courier.;
Payment of the application fee of €22.50.

 

C. Acknowledgment of Paternity

Requirements:

Personal appearance of both parents;
Four sets of Affidavit to Use the Surname of Father duly executed by the father;
Original and 4 photocopies of PSA issued Birth Certificate duly authenticated by the Department of Foreign Affairs;
Original and 4 photocopies of valid passports of parents;
Payment of the application fee of €22.50.
